I, the undersigned, as the parent or legal guardian of the participant named above, acknowledge that participation in middle school cheerleading involves inherent risks, including the risk of injury. I voluntarily assume all risks associated with my child's participation.
I hereby release, discharge, and hold harmless the school, its employees, coaches, and volunteers from any and all liability, claims, or demands arising from or related to participation in the cheerleading program.
I certify that my child is physically fit to participate in cheerleading activities.
