Waiver and Consent

I, the undersigned parent or legal guardian, give permission for the above-named minor to participate in dance classes, events, and activities. I understand that dance is a physical activity that carries an inherent risk of injury. In consideration of the minor’s participation, I agree to release, discharge, and hold harmless the dance studio, its instructors, employees, and volunteers from any and all claims, liability, or demands arising from any injury, illness, or accident that may occur during participation.

I further authorize the dance studio staff to act on my behalf in case of an emergency, including seeking medical treatment if necessary.
