I, the undersigned parent or guardian, acknowledge that participation in swimming activities involves inherent risks, including the risk of serious injury or death. By signing below, I consent to my child's use of the swimming pool and agree to assume all risks associated with such use.

I hereby release and hold harmless the facility, its owners, staff, and agents from any and all liability resulting from my child's participation in swimming pool activities. I confirm that my child is physically fit and able to participate, and I agree to abide by all rules and instructions provided by the facility.
