By signing below, I acknowledge that I am voluntarily participating in adult recreational basketball activities. I understand that participation may result in physical injury, and I assume all risks associated with this activity, including but not limited to, falls, contact with other participants, and the effects of weather, court conditions, and equipment.
I hereby release, waive, discharge, and agree to hold harmless the organizers, sponsors, facility owners, and their employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in or traveling to and from the activity.
