I, the undersigned, as the parent or legal guardian of the above-named participant, hereby acknowledge and agree that participation in the above sports activity involves potential risk of injury, including serious injury or death. I voluntarily accept and assume all such risks on behalf of my child.
I, on behalf of myself, my child, our heirs, executors, and assigns, hereby release, waive, and discharge the team/organization, its employees, coaches, volunteers, and agents from any and all claims, liabilities, or causes of action that may arise from participation in the activity, except for gross negligence or intentional misconduct.
I certify that my child is in good health and able to participate. I authorize emergency medical treatment if necessary.
