I, the undersigned, acknowledge and agree that I am renting the following costume(s) from the company on the dates specified below. I understand that I am responsible for returning all rental items in the same condition as received, and will be liable for any damage, loss, or excessive wear to the rental items beyond normal use.
I agree to reimburse the company for any repair costs, replacement costs, or cleaning fees, as assessed by the company, for items returned damaged, lost, or excessively soiled.
By signing below, I agree to all terms stated herein and accept full responsibility for all rented items.
