I acknowledge that I have received the listed rental items in good condition. I understand that I am responsible for any damage or loss of these items during the rental period. I agree to return all items in the same condition as received. If any item is damaged or lost, I will reimburse the replacement or repair cost as assessed by the rental provider. I have read and agree to the terms of this Damage/Loss Waiver.
