Equipment Rental Damage/Loss Liability Waiver

I, the undersigned, acknowledge that I am renting the equipment listed above and accept full responsibility for any damage, loss, theft, or destruction of the equipment during the rental period. I understand that I am liable for the cost of repair or replacement as determined by the equipment owner.

I agree to use the equipment in accordance with all instructions and safety guidelines, and not to allow unauthorized persons to use the equipment. I hereby waive any claims against the equipment owner for any injury, damage, or loss incurred while using the rented equipment.

I have inspected the equipment prior to rental and agree that it is in proper working condition.
