I, the undersigned dog owner, acknowledge and agree that I am solely responsible for any and all harm or damage caused by my dog while in the care, custody, or control of the dog walker. I hereby release, waive, and hold harmless the dog walker from any and all claims, causes of action, or liability of any sort arising from or related to the services provided, except in the case of willful misconduct or gross negligence.
I affirm that my dog is up to date on vaccinations and is not known to be aggressive. I agree to notify the dog walker of any behavioral or health issues.
