Agreement
I understand and agree to the following:
- I certify that my pet is healthy and up-to-date on vaccinations.
- I authorize the daycare provider to act in my pet’s best interest in case of emergency.
- I will inform the daycare of any behavioral or medical changes.
- I agree to pay all daycare fees as required.
