1. Owner certifies that the pet described above is in good health and has not been exposed to any contagious illnesses.
2. Owner shall be responsible for any property damage or personal injury caused by their pet.
3. Owner authorizes the boarding facility to seek veterinary care if necessary and agrees to pay for any associated costs.
4. The boarding facility is not liable for illness or injury provided reasonable care and precautions are taken.
