This Uniform Maintenance Responsibility Waiver ("Waiver") is entered into by and between:
The employee acknowledges receipt of the following uniform(s):
The employee acknowledges and agrees to be fully responsible for the care, cleaning, and maintenance of the issued uniform(s) during the duration of employment.
The employer is released from any obligation to provide cleaning, laundry, repair, or replacement of the issued uniform(s), except as required by law or stated in the company policy.
Upon termination of employment or request, the employee agrees to return the issued uniform(s) in good condition, reasonable wear and tear excepted.
By signing below, the employee acknowledges understanding and acceptance of the terms described in this waiver.
