Surgical Patient Confidentiality Agreement

This Confidentiality Agreement ("Agreement") is entered into between:

1. Purpose

The purpose of this Agreement is to protect the confidentiality and privacy of all surgical patients and their personal, medical, and health information, in accordance with applicable laws and institutional policies.

2. Confidential Information

"Confidential Information" refers to any and all information related to patients, including but not limited to:

• Patient identity
• Medical records and history
• Surgical procedures performed
• Personal information, diagnosis, and treatment

3. Obligations

• Access confidential information only as required to perform my designated duties.
• Not disclose confidential information to any unauthorized person or entity.
• Secure and safeguard all patient information at all times.
• Comply with all relevant laws, regulations, and hospital policies regarding patient confidentiality.

4. Duration

The obligations under this Agreement remain in effect at all times during and after the period of employment or provision of services.

5. Acknowledgment

I acknowledge that failure to comply with this Agreement may result in disciplinary action, termination, or legal consequences.
