High Fashion Editorial Model Consent Form

I, the undersigned, consent to participate as a model in the high fashion editorial project described above. I authorize the use of my images, likeness, and name for editorial, promotional, and commercial purposes in any medium or format, worldwide, in perpetuity. I understand that my participation is voluntary and that I will not receive further compensation beyond what is agreed. I confirm that I am at least 18 years old, or have guardian permission if under 18.
