I, the undersigned model, hereby grant permission to the fashion designer named below to use photographs and/or video images taken of me for any lawful purpose, including but not limited to: publication, promotion, illustration, advertising, and web content.
I understand that all images and videos taken during the shoot are the property of the designer mentioned above and may be used as described. I confirm that I am over the age of 18, or have consent from my legal guardian (if under 18).
