I, the undersigned Volunteer, acknowledge and agree that my participation in the above activity is voluntary, and that I assume all risks associated with such participation, including but not limited to injury, illness, loss, or property damage.

I hereby release, discharge, and hold harmless

as well as their officers, employees, and agents from and against any and all claims, damages, or liabilities that may arise as a result of my participation in this activity.

Medical Treatment

I give permission for emergency medical treatment if necessary. I acknowledge that I am responsible for my own medical insurance and any costs that may arise from medical treatment.
