I hereby acknowledge and agree that I am voluntarily placing my pet under the care of the boarding facility. I release the facility, its owners, employees, and agents from any and all liability for injuries or damages resulting from my pet's participation in boarding activities, except due to gross negligence or intentional misconduct. I confirm that my pet is current on required vaccinations and that I have disclosed any and all behavioral or medical issues.
I assume full responsibility for any harm caused by my pet to people, other animals, or property while boarded. In the event of a medical emergency, I authorize the facility to seek veterinary care for my pet as needed, and agree to be financially responsible for such care.
