I, the undersigned model, hereby grant the photographer permission to photograph me and use my likeness in all forms and media for advertising, trade, promotional, and any other lawful purposes. I waive the right to inspect or approve the finished product wherein my likeness appears. I understand that I do not own the copyright of any image taken by the photographer.
