Telemedicine Confidentiality Agreement

1. Definition of Confidential Information

For the purpose of this Agreement, "Confidential Information" includes all patient health information, medical records, case files, communications, and any other information exchanged during or in relation to telemedicine services, whether oral, written, or electronic.

2. Obligations

• All Confidential Information must be kept strictly confidential.
• No Confidential Information shall be disclosed to any third party without prior written consent.
• Confidential Information will only be used for the purpose of providing telemedicine services.

3. Exceptions

• If required by law or court order, the Recipient may disclose Confidential Information after providing notice to the Provider.
• Information that has become public through no breach of this Agreement is not subject to confidentiality obligations.

4. Term

This Agreement shall remain in effect for the duration of the telemedicine relationship and continue as long as required by law.

5. Acknowledgement

The parties have read, understood, and agreed to abide by the terms of this Confidentiality Agreement.
